Navigated ultrasound-based image guidance during resection of gliomas: practical utility in intraoperative decision-making and outcomes

چکیده
OBJECTIVE Intraoperative imaging is increasingly being used for resection control in diffuse gliomas, which the extent of (EOR) important. ultrasound (iUS) has emerged as a highly effective tool this context. Navigated (NUS) combines benefits real-time with navigation guidance. In study, authors investigated use NUS an intraoperative adjunct gliomas. METHODS The retrospectively analyzed 210 glioma patients who underwent surgery using at their center. analysis included decision-making, diagnostic accuracy, and operative outcomes, particularly EOR related factors influencing this. RESULTS US-defined gross-total (GTR) was achieved 57.6% patients. Intermediate scans were evaluable 115 instances. These prompted change decision 42.5% cases (the majority further unanticipated residual tumor). Eventual MRI-defined GTR rates similar (58.6%), although concordance between US MRI 81% (170/210 cases). There 21 false positives 19 negatives NUS, resulting sensitivity 78%, specificity 83%, positive predictive value 77%, negative 84%. A large proportion (13/19 patients, 68%) false-negative results eventually had near-total resections. Tumor resectability, delineation, enhancement pattern, eloquent location, image resolution significantly influenced rate, though only resectability location significant on multivariate analysis. CONCLUSIONS useful detecting tumor residues positively course resection, leading to higher rates. Nevertheless, determined by innate its relationship reinforcing need combine iUS functional mapping techniques optimize

Decision making and management of gliomas: practical considerations.
Over the last decade, diagnostic options and introduction of novel treatments have expanded the armamentarium in the management of malignant glioma. Combined chemoradiotherapy has become the standard of care in glioblastoma up to the age of 70 years, while treatment in elderly patients or with lower grade glioma is less well defined. Molecular markers define different disease subtypes and allow...
متن کاملImage guidance in skull base tumor resection: A synergistic approach using intraoperative navigated angiosonography for real-time vessel visualization
The authors report their experience with image-based preoperative vascular and neural element segmentation with three-dimensional (3D) reconstruction for intraoperative guidance in ten patients undergoing surgery for skull base tumor resection. In all cases, the 3D segmented structures were correlated to the correspondent intraoperative findings with the aid of neuronavigation. They conclude th...

ince the first recorded use of the “ultrasonoscope” for localizing subcortical intracerebral neoplasms in postmorS tem specimens in 1950 (4), technological advances in ultrasound have seen a transformation from poor-quality, A-mode wave forms to superb-quality, real-time images in 2 (2D) and 3 (3D) dimensions. As a result, intraoperative ultrasonography (IOUS) has undergone remarkable developme...
متن کاملAuditory support for resection guidance in navigated liver surgery.
BACKGROUND An alternative mode of interaction with navigation systems for open liver surgery was requested. Surgeons who use such systems are impeded by having to constantly switch between viewing the navigation system screen and the patient during an operation. METHODS To this end, an auditory display system for open liver surgery is introduced with support for guiding the tracked instrument...
متن کاملIntraoperative navigated 3-dimensional ultrasound angiography in tumor surgery.
BACKGROUND Avoiding damage to blood vessels is often the concern of the neurosurgeon during tumor surgery. Using angiographic image data in neuronavigation may be useful in cases where vascular anatomy is of special interest. Since 2003, we have routinely used 3D ultrasound angiography in tumor surgery, and between January 2003 and May 2005, 62 patients with different tumors have been operated ...
